Short answer

Why does a late design increase the cost?

The fair date is fixed. When approval slips, the lost time comes out of production, not installation. A shortened production window costs money in four places: weekend and night shifts in the workshop, express or air freight instead of road, whatever material is in stock instead of the one specified, and work finished on site because it could not be finished in the workshop. Together they cost far more than approving the design two weeks earlier would have.
